WORKFORCE DEVELOPMENT

As a Minority Business Enterprise (MBE), Changing Lives at Home Mental Health Services Inc has partnered with Baltimore City Community College to recruit, individuals for, occupational training; industry certifications; and lifelong learning opportunities.

During and after enrollment in this 3-week program, these individuals are partnered with a Career Advocate who will walk alongside them during their career journey to encourage, support and direct each individual and help them in developing a strong career foundation . Upon successful completion of the program, our staff assists with job placement and continues to work with individuals as they begin life in a new career.

 

VISION

Changing Lives at Home,Inc. Group Home (CLH) is a licensed community-based 5-bed residential group home that provides therapeutic services and 24 hour staffing to adolescent females with behavioral, emotional, and mental health difficulties ages 15-19 located in Baltimore City. CLH is licensed through Department Of Human Resources and accredited by The Joint Commission. Changing Lives at Home accepts referrals from Baltimore City and all surrounding Maryland Counties Department of Social Services.

Changing Lives at Home, Inc. works closely with families and referring agencies for reunification. Our dedicated work force provides diligent supervision of the residents under our care by providing individualized services to the female youth via the individual plan of care. Youth enrolled in the program are required to attend school, participate in structured recreation programs, attend individual and group therapy and engage in pro-social behaviors.

PSYCHIATRIC REHABILITATION PROGRAM (PRP)

CLHMHS Psychiatric Rehabilitation Program (PRP) provides all persons and family systems, community-based assistance to optimize their overall wellness and quality of life with the goal of rehabilitation and recovery. Our organization promotes the healthy restoration of the individual’s deficiencies by assisting them in the development of their life skills. CLHMHS’ primary focus is to empower the client and assist the individual to identify personal goals and overcome objectives and work towards a full recovery.

 

SUBOXONE

Suboxone contains a combination of buprenorphine and naloxone. Buprenorphine is an opioid medication, sometimes called a narcotic. Naloxone blocks the effects of opioid medication, including pain relief or feelings of well-being that can lead to opioid abuse. It is used to treat opiate addiction and dependence.  Suboxone, along with treatment from Changing Lives at Home Mental Health Services, allows better adherence to using opiates. Suboxone is recommended to be only used in accordance with a comprehensive treatment program. The drug can produce dependence in users, so it’s important that all taking the medication remain under professional medical supervision, which CLHMHS provides.

Changing Lives at Home Mental Health Services provides comprehensive behavioral health services that are designed to meet the needs of individual consumers in the home, school, and community. These services are listed above.

MEDICATION MANAGEMENT

Medication Management is a cumulative effort between medical professionals and consumers to ensure that drugs are being used as prescribed while limiting chances for abuse and to ensure that consumers are educated about not only their mental illness, but the drug they are  prescribed to treat such mental illness.

When providing Med Management services, Medical Professionals use treatment such as:

  • Performing patient assessments
  • Formulating short and long term med treatment plans
  • Ensuring compliance through patient education

Documenting and communicating between medical professionals and consumers to provide high quality care
