From mental health treatment and psychiatric rehabilitation to care coordination, medication management, residential support, and recovery services, our programs are designed to help individuals and families improve their well-being and work toward meaningful goals.

OUR SERVICE PROVIDERS & PARTNERS

Small Residential Group Home

A Safe, Supportive Place to Grow.

Our licensed 5-bed community-based residential group home provides therapeutic services and 24-hour staffing for adolescent females ages 15–19 experiencing behavioral, emotional, or mental health challenges. Residents receive individualized support based on their individual plan of care while participating in school, therapy, structured recreation, life-skills development, and positive social activities. CLH works closely with families and referring agencies to support each young person's development, well-being, and long-term goals.

Psychiatric Rehabilitation Program (PRP)

Building Skills. Building Confidence. Building Independence.

Our Psychiatric Rehabilitation Program (PRP) provides community-based support to help individuals develop the skills they need to manage everyday life and work toward greater independence. Through individualized support, consumers can work on personal goals, life skills, social skills, community integration, wellness, and recovery. Our goal is to empower each individual to recognize their strengths, overcome challenges, and make meaningful progress toward the life they want.

Medication Management

Medication Support With Your Care in Mind.

Medication can be an important part of an individual's overall behavioral health treatment. Our medication management services support individuals in understanding and safely managing their prescribed medications as part of their treatment plan. Our professionals work with consumers to: • Assess individual needs • Develop medication treatment plans • Provide education about medications and behavioral health • Promote medication adherence • Coordinate care with other healthcare professionals Our goal is to support informed decisions, safe treatment, and continuity of care.

Health Home

Coordinated Care for Better Health.

Our Health Home program provides enhanced care coordination for eligible individuals living with chronic health conditions. The program takes a community-based, person-centered approach to help individuals better manage their health and connect with the services and resources they need. Health Home support may include coordination with behavioral health services, healthcare providers, community resources, and other supports. Our goal is to help individuals better manage their health, improve outcomes, and access the care and resources they need.

Outpatient Mental Health Services

A Safe Space to Talk, Heal, and Grow.

Our outpatient program provides comprehensive behavioral health services designed around the needs of each individual. Services may include: • Mental health and substance use diagnostic assessments • Individual therapy • Family therapy • Group therapy • Case management • Psychiatric evaluations • Medication management • Psychoeducation • Referrals for psychological evaluation and testing • Collaboration with other medical and healthcare providers Our goal is to provide individuals with the support, tools, and resources they need to work toward improved emotional and behavioral well-being.

Substance Use Disorder Program

Support for Recovery. Support for a Better Tomorrow.

Recovery is a journey, and the right support can make a difference. CLH provides substance use disorder services designed to help individuals build healthier habits, strengthen recovery skills, and work toward long-term wellness. Services may include: • DWI/DUI education • Individual therapy • Group therapy • 12-step meetings • Relapse prevention education • Strategies for managing urges and cravings • Healthy life-skills development • Long-term recovery support • Support for rebuilding personal and family relationships CLH also provides office-based treatment for opioid dependence through medication-assisted treatment under professional medical supervision.

Discounted / Sliding Fee Program

Quality Care Should Be Accessible.

Financial challenges should not prevent anyone from seeking support. CLH offers a discounted/sliding fee program for eligible individuals who are uninsured, underinsured, or experiencing financial hardship. Eligibility and fees are based on household income and size, with documentation reviewed according to program guidelines.
