BUPRENORPHINE MAINTENANCE

Suboxone contains a combination of buprenorphine and naloxone. Buprenorphine is an opioid medication, sometimes called a narcotic. Naloxone blocks the effects of opioid medication, including pain relief or feelings of well-being that can lead to opioid abuse. It is used to treat opiate addiction and dependence. Suboxone, along with treatment from Changing Lives at Home Mental Health Services, allows better adherence to using opiates. Suboxone is recommended to be only used in accordance with a comprehensive treatment program. The drug can produce dependence in users, so it’s important that all taking the medication remain under professional medical supervision, which CLHMHS provides.

 

MEDICATION MANAGEMENT

Medication Management is a cumulative effort between medical professionals and consumers to ensure that drugs are being used as prescribed while limiting chances for abuse and to ensure that consumers are educated about not only their mental illness, but the drug they are prescribed to treat such mental illness.

When providing Med Management services, Medical Professionals use treatment such as:

Performing patient assessments
Formulating short and long term med treatment plans
Ensuring compliance through patient education

Documenting and communicating between medical professionals and consumers to provide high quality care

 

HEALTH HOME

Health Home is a service provided by Changing Lives at Home Inc, Mental Health to individuals with chronic conditions that are unable to leave their home easily to access somatic and behaviorial health services. The Health Home program offers its participants enhanced care coordination services including psychiatric rehabilitation, mobile treatment and opiod treatment services. Health Homes is designed to provide a community based approach to enhance person-centered care and empower participants to manage and prevent chronic conditions and improve health outcomes while avoiding preventable hospital encounters.
