SMALL RESIDENTIAL GROUP HOME

Changing Lives at Home Mental Health Inc. (CLHMH) Group Home is a licensed community-based 5-bed residential group home that provides therapeutic services and 24-hour staffing to adolescent females with behavioral, emotional, and mental health difficulties ages 15-19 located in Baltimore City. CLH is licensed through Department Of Human Resources and accredited by The Joint Commission. Changing Lives at Home accepts referrals from Baltimore City and all surrounding Maryland Counties Department of Social Services.

Changing Lives at Home Mental Health Inc. (CLHMH) works closely with families and referring agencies for reunification. Our dedicated workforce provides diligent supervision of the residents under our care by providing individualized services to the female youth via the individual plan of care. Youth enrolled in the program are required to attend school, participate in structured recreation programs, attend individual and group therapy and engage in pro-social behaviors.

PSYCHIATRIC REHABILITATION PROGRAM (PRP)

CLHMH Psychiatric Rehabilitation Program (PRP) provides all persons and family systems, community-based assistance to optimize their overall wellness and quality of life with the goal of rehabilitation and recovery. Our organization promotes the healthy restoration of the individual’s deficiencies by assisting them in the development of their life skills. CLHMHS’ primary focus is to empower the client and assist the individual to identify personal goals and overcome objectives and work towards a full recovery.

BUPRENORPHINE MAINTENANCE

Suboxone contains a combination of buprenorphine and naloxone. Buprenorphine is an opioid medication, sometimes called a narcotic. Naloxone blocks the effects of opioid medication, including pain relief or feelings of well-being that can lead to opioid abuse. It is used to treat opiate addiction and dependence. Suboxone, along with treatment from Changing Lives at Home Mental Health, Inc. allows better adherence to using opiates. Suboxone is recommended to be only used in accordance with a comprehensive treatment program. The drug can produce dependence in users, so it’s important that all taking the medication remain under professional medical supervision, which CLHMHS provides.

MEDICATION MANAGEMENT

Medication Management is a cumulative effort between medical professionals and consumers to ensure that drugs are being used as prescribed while limiting chances for abuse and to ensure that consumers are educated about not only their mental illness, but the drug they are prescribed to treat such mental illness.

When providing Med Management services, Medical Professionals use treatment such as:

Performing patient assessments
Formulating short and long term med treatment plans
Ensuring compliance through patient education

Documenting and communicating between medical professionals and consumers to provide high quality care
